- ItemAcesso aberto (Open Access)Achados oculares em crianças de zero a seis anos de idade, residentes na cidade de São Caetano do Sul, SP(Conselho Brasileiro de Oftalmologia, 2003-12-01) Beer, Sandra Maria Canelas [UNIFESP]; Scarpi, Marinho Jorge [UNIFESP]; Minello, Antonieta Antunes [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)PURPOSE: To establish the prevalence of refractive errors, strabismus, amblyopia and anisometropia, among children in a population, using two methods of screening (at school and at the National Polio Vaccination Day). METHODS: We examined 2,640 children, between 0 and 6 years of age. The children, residents of the city São Caetano do Sul, were divided into two groups (A and B). Group A: 476 children, with an age range of a few months to 6 years, had their eyes examined by 10 ophthalmologists and 11 ophthalmology technicians, during the National Polio Vaccination Day (June 20,1998). Group B: 2,164 children attending municipal schools (EMEIs and EMIs), between 4 and 6 years of age, who had their eyes examined by two ophthalmologists during the year of 1998. RESULTS: In group A, the prevalence of strabismus was 3.36% and that of anisometropia was 1.26%. The most common type of refractive error was hypermetropia £ 2D and 14.11% required optical prescription. In group B, the prevalence of strabismus was 1.43%. In the group who failed in the screening, the prevalence of amblyopia was 1.39% and that of anisometropia was 2.8%. The most common type of refractive error found was hypermetropic astigmatism and 56.48% required optical prescription. CONCLUSION: The prevalence of the ocular findings in children in São Caetano do Sul, although being a city with a privileged socioeconomic profile, is similar to that reported in the literature. Ophthalmic examination on National Polio Vaccination Day showed to be an efficient screening tool.

Methods: A retrospective study of 72 patients with myelomeningocele was performed to evaluate ocular motility disorders, refractive errors and indirect ophthalmoscopy findings. Results: In a group of 72 patients with myelomeningocele, aged 5 month -- 18 years, 64 (88.9%) had hydrocephalus. Of this group of 64 patients, 36 (56.3%) had strabismus, 21 (58.3%) of whom were esotropic, and 13 (36.1%) exotropic, 1 (2.8%) exophoric and 1 (2.8%) presented sometimes esotropia and sometimes exotropia. Of the total of 72 studied patients, 38 (52.8%) had strabismus. Anisotropia was found in 16 (22.2%) patients, 14 (87.5%) with an A pattern and 2 (12.5%) with a V pattern. The most frequent refractive error was hyperopia, found in 64 (44.4%) eyes. All the 16 patients with anisotropia had hydrocephalus. Atrophy of the optic nerve was found in 9 (12.5%) patients. Conclusion: Myelomeningocele associated with hydrocephalus had a higher percentage of strabismus than the normal population.

Methods: A reliability analysis at an institutional practice performed in adult patients scheduled for strabismus surgery on rectus muscles. The distance between the EOM insertion and the limbus was measured preoperatively with AS-OCT. The value was compared to intraoperative measurement obtained with calipers. Additional measurements included the limbus-anterior chamber angle distance with AS-OCT and the axial length with IOLMaster. The main outcome measure was the greement between preoperative AS-OCT and intraoperative measurements. A difference of ≤ 1 millimeter (mm) was "clinically acceptable". Results: 144 muscles were analyzed from 74 patients. 31 of 33 reoperated muscles were successfully imaged. AS-OCT measurements were within 1 mm of intraoperative measurements in 77% of all the muscles. The accuracy was higher for muscles with no prior surgery (83%), as compared to re-operated muscles (58%). Although the accuracy decreased when comparing reoperations to primary surgeries for the medial (79 to 63%; p=0.09; 95%CI, -1.38 to 0.11) and the lateral rectus (81 to 49%; p=0.11; 95%CI, -2.06 to 0.22), the difference was not significant. No correlation between limbus-anterior chamber angle distance and axial length was established. Conclusions: AS-OCT is valuable in identifying EOM insertions in primary strabismus surgeries, but the accuracy decreases in re-operations. The intraoperative measure performed before the detachment of the muscle's tendon and the use of limbus as a reference point in the OCT proved to be suitable for performing the measurements.

- ItemSomente MetadadadosAlteracoes oculares em criancas expostas ao virus da imunodefiCiência humana(Universidade Federal de São Paulo (UNIFESP), 1997) Nakanami, Celia Regina [UNIFESP]Foram estudadas 109 criancas soropositivas para o HIV, das quais 104 eram nascidas de maes infectadas e 5 adquiriram o virus por transfusao e sangue ou derivados contaminados, com os objetivos de descrever, e comparar as alteracoes oculares encontradas nesses pacientes, alem de estudar a relacao desses achados com a presenca de infeccao pelo HIV e o estado imunologico dos pacientes. Os pacientes foram divididos em dois grupos: grupo SR, de criancas HIV negativas (controle) que sofreram exposicao ao virus materno e eram apenas portadoras de anticorpos maternos transmitidos passivamente no periodo gestacional ou de trabalho de parto e nao foram infectadas pelo HIV; e grupo I, daquelas que desenvolveram infeccao pelo HIV, apresentando ou nao doenca definidora de AIDS. No grupo SR foram agrupados 29 pacientes; e no grupo I, 80. Todas as criancas foram referidas para exame ocular pelos centros de atendimento de doencas infecto-contagiosas/sexualmente transmitidas, apos exames pediatrico e laboratoriais, inclusive com CD4. O exame ocular revelou que das 109 criancas menores de 13 anos expostas ao HIV, 60 (54,13%) tinham exame normal, 49 (45,87%) apresentaram alteracoes oculares e dentre elas, 35 (32,11%) apresentavam ceratoconjuntivite sicca; 5 (4,60%), com blefarite; um (0,92%) paciente com calazio; um (0,92%) com herpes zoster oftalmico; 5 (4,60%), toxoplasmose ocular; 5 (4,60%), perivasculite retiniana; 8 (7,34%), com exsudatos algodonosos; estrabismo; 3 (2,75%) e 4 (3,70%) com ambliopia. Os achados deste estudo e os resultados estatisticos mostram que as alteracoes oculares ocorrem com mais frequencia nos pacientes infectados pelo HIV e sugere-se tambem que ocorram naqueles individuos com imunossupressao moderada ou grave

METHODS: Eight patients (five female) were enrolled to measure ocular motility prior and 1, 7, 30 and 180 days after one injection of 2 ml of 1.5% bupivacaine and 2.5 U of botulinum toxin A in agonist and antagonist muscles, respectively, of eight amblyopic eyes. Muscle thickness was measured prior and on days 1, 7 and 30 after injection using 10-MHz ultrasonography (eyelid technique). RESULTS: Mean change in alignment was 10 prism diopters after 180 days (n=6). An average increase of 1.01 mm in muscle thickness was observed after 30 days of bupivacaine injection and 0.28 mm increase was observed after botulinum toxin A injection, as measured by ultrasonography. Lateral rectus muscles injected with bupivacaine had a mean increase of 1.5 mm in muscle thickness. CONCLUSION: In this study, a change in ocular motility was observed after 180 days of intramuscular injection of bupivacaine and botulinum toxin in horizontal extraocular muscles. Overall, there was an increase of muscle thickness in both botulinum toxinum A and bupivacaine injected muscles after 30 days of injection when measured by ultrasonography. This change was more pronounced on lateral rectus muscles after bupivacaine injection.

METHODS: Twenty-nine (29) patients with strabismus and low visual acuity in one eye were examined at the Department of Ophthalmology of UNIFESP. They were divided into 2 groups - group I with 17 patients receiving the botulinum toxin A injection using Mendonça's forceps, and group II with 12 patients receiving the toxin with electromyographical guidance. The patients of both groups were examined on the 7th and 14th day after intervention and the outcome of both methods of botulinum toxin A injection were compared. Friedman and Mann-Whitney correlation tests were used in the statistical analysis of the data. RESULTS: Although the follow-up examinations on the 7th and 14th days showed a different behavior between both methods, there was no statistically significant difference between the mean values of the correction attained in both groups at the end of this study. CONCLUSION: No statistically significant difference was shown between the two groups. Mendonça's forceps can be a safe alternative to electromyography to locate an extraocular muscle for botulinum toxin A injection.

Methods: A retrospective chart review of 24 patients with esotropia and high myopia (greater than -6.00 spherical diopters) and 17 patients with exotropia and high myopia compared with their control groups. Deviation between 10delta of esotropia and 10delta of exotropia was considered a surgical success. Results: We noticed a high incidence of bad results among the esotropic patients with high myopia. Conclusion: There is a tendency of bad results among the esotropic patients with myopia greater than -6.00 when compared with patients with spherical refractive error between -0.75 and +3.50.

Overaction of the superior oblique muscle (SO) results in hypotropia (HoT) in primary position of gaze (PP). The aim of this study was to evaluate the correction of HoT in PP after unilateral tenectomy of the superior oblique muscle close to its scleral insertion. Patients and Methods: We retrospectively studied, from 1977 until 1996, charts of 15 patients with SO overaction and hypotropia in PP greater than 4D who underwent unilateral superior oblique tenectomy close to its insertion at Hospital Santa Casa de São Paulo (12 patients), University of Santo Amaro (2 patients) and private clinic of one of the authors (CSD, 1 patient). The mean preoperative deviation in PP was 9D. The mean preoperative superior oblique overaction was 1.7 (measured by a cross system rating). Results: The mean correction of the HoT was 4.67D ± 5.09D (-5D to 15D), (H = 6.032; p = 0.014). The mean correction of SO overaction was 0.87 ± 0.88 + (0 - 2). According to the horizontal deviation, ET and XT, there was no significant mdifference in the correction of the HoT. Conclusion: For HoT of 15D or less we found a mean correction of 51.82% of its preoperative value. Unilateral tenectomy has shown to be an effective technique for the correction of vertical deviation in PP in our study.
- ItemSomente MetadadadosCusto do tratamento oclusivo em pacientes do setor de estrabismo da disciplina de oftalmologia da Faculdade de Medicina da Fundacao Universitaria do ABC(Universidade Federal de São Paulo (UNIFESP), 2003) Fernandez, Celso Lopez [UNIFESP]Objetivo: Avaliar o custo do tratamento oclusivo, em pacientes do Setor de Estrabismo da Disciplina de Oftalmologia da Faculdade de Medicina da Fundacao Universitaria do ABC, analisando a situacao socio-economica dos pacientes e a adesao a terapeutica oclusiva. Metodos: Atraves de questionario aplicado a 51 pacientes submetidos a tratamento oclusivo e a seus responsaveis legais, realizou-se estudo transversal, onde foram investigados : motivo da oclusao, escolaridade do paciente e familiar, atividade profissional, renda familiar, tempo de tratamento e custos com a compra de oclusores. A populacao estudada foi dividida em 3 grupos de acordo com a renda familiar, avaliando-se' i a dificuldade economica de cada grupo, em custear o tratamento oclusivo. Resultados: O custo mensal medio para adquirir oclusores foi de 21,93 :t 11,63 reais. Metade da populacao de menor renda, deixou de adquirir oclusores ao menos uma vez' durante o tratamento, por motivos financeiros (p=0,04). Neste grupo populacional houve um comprometimento de 12 por cento ou mais da renda familiar, destinada a compra de oclusores. Conclusao: Concluiu-se que o custo dos oclusores pode ser um fator limitante na adesao i ao tratamento oclusivo, principalmente em populacoes de baixa renda, comprometendo desta forma o sucesso do tratamento proposto
- ItemAcesso aberto (Open Access)Estudo comparativo da eficácia do uso de prednisolona e cetorolaco de trometamina tópicos após cirurgia de estrabismo(Conselho Brasileiro de Oftalmologia, 2003-08-01) Meireles-teixeira, Jorge [UNIFESP]; Moya, Marcos Pitarello [UNIFESP]; Cuperman, Jairo [UNIFESP]; Mendonça, Tomás [UNIFESP]; Nakanami, Célia [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)PURPOSE: To compare the use of non-steroid anti-inflammatory drugs (NSAID) with steroids in the follow-up of strabismus surgery, regarding inflammation control. METHODS: All patients underwent resection-recession surgery. Both groups used the same antibiotic eye-drop. One group used NSAID (ketorolac tromethamine- Acular®) and the other, prednisolone, 0.12% - Pred Mild®, for 21 days. They were examined with slit-lamp and ectoscopy to evaluate IOP (comparing with the fellow eye), edema, hyperemia and comfort. RESULTS: Of a total of 23 patients, 15 were treated with NSAID and 12 with steroid. Regarding comfort, edema and IOP there was no important difference between the NSAID and steroid groups. However, when we analyzing hyperemia, there were 5 (33.3%) patients in the NSAID group with ++/4 of hyperemia at the 21st postoperative day and only 1 (8.3%) patient in the steroid group. There were also three granulomas (20%) in the NSAID group and none in steroid-treated patients. CONCLUSION: For resection-recession surgeries, procedures that induce a greater inflammatory response, the use of NSAID is not advisable.

Methods: Ophthalmic and orthoptic evaluations were performed prospectively on 110 strabismic probands and 478 relatives. We used 3 different criteria in the diagnosis of strabismus: primary diagnosis (dx1) defined as any manifest horizontal or vertical deviation, a secondary diagnosis (dx2) including esophoria (>7 prism diopters) or exophoria (>9 prism diopters), and a tertiary diagnosis (dx3) including abnormal fusional amplitudes, accommodative convergence/accommodation (AC/A) ratio, and/or stereopsis; monofixation syndrome; 4 prism diopters base out; and/or abnormal Maddox test responses. Analyses were carried out within mating types. Results: Hypotheses of autosomal dominant or recessive inheritance with no sporadics were rejected. Based on the dx1, 25% of the families had more than one individual affected and there was vertical transmission in 13%; adding dx2 there were 36% of the families with more than one affected and 21% had vertical transmission; and adding dx3, there were 73% with more than one affected and 51% with vertical transmission. Conclusions: There is evidence for a pattern consistent with an autosomal dominant form of strabismus in most families.

Methods: 42 eyes of 21 patients with acquired moderate angle esotropia without any other form of strabismus were prospectively enrolled and submitted to a complete ophthalmological examination followed by recess/resect procedure in a single eye. Data from the fellow eye were selected as control. Ophthalmological assessment was carried out preoperatively, 1 month after surgery and 6 months after surgery, where astigmatism was compared using vector analysis and the polar value concept. Results: The eyes submitted to surgery revealed a significant (p<0.05) decrease in spherical equivalent, from 3.28 ± 1.98 diopters to 3.05 ± 1.95 diopters. Refraction data disclosed a significant increase in the 90° component of net astigmatism, from 0.458 ± 0.594 diopters to 1.002 ± 0.718 diopters, which was also observed in keratometric readings:1.083 ± 0.560 diopters to 1.690 ± 0.591 diopters. Surgically induced astigmatism, assessed using refraction data was 0.63 ± 0.27 diopters at an average axis of 92.30 ± 14.91 degrees, and 0.71 ± 0.27 diopters at an average axis of 94.45 ± 15.69 degrees as obtained by keratometric readings. This is visually demonstrated by the corneal topography difference map. Conclusions: There is a statistically significant and clinically relevant increase in with-the-rule astigmatism in esotropic patients submitted to monocular recess/resect surgery. This change is stable at a 6 month follow-up and is associated with a decrease in mean spherical equivalent.

Método: Estudo retrospectivo dos prontuários de crianças atendidas no Ambulatório de Catarata Congênita do Departamento de Oftalmologia da Universidade Federal de São Paulo no período entre 2001 e 2011. Foram incluídos pacientes com diagnóstico de catarata congênita ou de desenvolvimento. Foram excluídos os pacientes com catarata traumática; secundárias a uveíte, radiação ou medicamentos; pacientes operados em outro serviço; pacientes com glaucoma; leucocorias não cristalinianas (retinoblastoma, retinopatia da prematuridade, leucocorias pré-cristalinianas), e com sub-luxação do cristalino. Foram avaliadas: as frequências cronológicas, etiológicas e morfológicas das cataratas; a lateralidade e a ocorrência de estrabismo associado nestes pacientes. Resultados: Foram incluídos 207 pacientes. Cento e dezessete (56,5%) apresentavam catarata congênita e 90 (43,5%) apresentavam catarata de desenvolvimento. Cento e nove (52,6%) pacientes eram portadores de catarata unilateral. Quanto à morfologia, 72 crianças (33,8%) apresentavam catarata zonular e 66 (31,9%) apresentavam catarata total. A etiologia idiopática foi a mais frequente (72,5%) afetando 150 pacientes. Foram observados 108 pacientes (52,2%) com estrabismo associado, especialmente endotropias secundárias. Conclusões: A etiologia idiopática foi a mais frequente neste estudo. O tipo morfológico zonular foi o mais frequentemente diagnosticado. Cataratas unilaterais ocorreram mais frequentemente em pacientes com persistência da vasculatura fetal. O estrabismo associado ocorreu em 52% dos pacientes. A análise dos resultados deste estudo pode contribuir para diagnosticar a catarata pediátrica precocemente e de modo mais preciso.
- ItemSomente MetadadadosPesquisa operacional em saúde ocular em região de alto analfabetismo da Amazônia: detecção dos agravos e intervenção nas ametropias(Universidade Federal de São Paulo (UNIFESP), 2000) Branco, Bruno Castelo [UNIFESP]; Chamon, Wallace [UNIFESP]O presente trabalho e resultado de tres visitas, realizadas entre 1997 e 1999 para o municipio de Pauini, que esta localizado no Sudoeste do estado do Amazonas. Como um estudo de pesquisa operacional, tres fases foram planejadas: (a) deteccao das alteracoes oculares da populacao, (b) planejamento e intervencao na Saúde ocular e (c) avaliacao dos resultados obtidos com as intervencoes realizadas. Na deteccao das alteracoes oculares foram avaliados 524 moradores do municipio que tem uma populacao de aproximadamente 17.000 habitantes. O erro refrativo foi a alteracao mais prevalente, e principal responsavel pelos casos de visao baixa (42,1 por cento), estando a hipermetropia e a miopia presentes em 30,3 por cento e 16,6 por cento da populacao. Quarenta e nove pacientes (9,3 por cento) apresentavam pterigio e 36 (6,9 por cento) apresentavam catarata. A catarata foi a causa mais frequente de cegueira e a principal causa de visao baixa, sendo responsavel por 73,3 por cento e 29 por cento dos casos, respectivamente. Os autores selecionaram 49 pacientes para um total de 60 cirurgias, do quais, 44 olhos, que, posteriormente, foram submetidos a cirurgia de catarata. Baseado na refracao automatica, foram prescritos, e doados 230 pares de oculos para 175 pacientes (33,14 por cento). A intervencao pela prescricao dos oculos para perto foi efetiva na melhora da acuidade visual (p<0,00l). Na avaliacao subjetiva dos oculos para perto 93,9 por cento apresentavam satisfacao entre boa e regular. Com correcao para longe 32 (74,4 por cento) apresentaram acuidade visual igual ou melhor que O,8, menor, entretanto, que a acuidade visual com correcao acrescida do buraco estenopeico, onde 38 (88,4 por cento) apresentaram acuidade visual de O,8 ou melhor. O aumento da acuidade visual com o buraco estenopeico foi significante (p< O,001). METHODS: This study was carried out during the Annual Meeting of the Brazilian Möbius Society in November 2008. Forty-four patients diagnosed with the Möbius sequence were submitted to a comprehensive assessment, on the following specialties: ophthalmology, neurology, genetics, psychiatry, psychology and dentistry. Forty-three patients were cooperative and able to undertake the ophthalmological examination. Twenty-two (51.2 %) were male and 21 (48.8%) were female. The average age was 8.3 years (from 2 to 17 years). The visual acuity was evaluated using a retro-illuminated logMAR chart in cooperative patients. All children were submitted to exams on ocular motility, cyclopegic refraction, and fundus examination. RESULTS: From the total of 85 eyes, using the spherical equivalent, the major of the eyes (57.6%) were emmetropics (>-0.50 D and <+2.00 D). The prevalence of astigmatism greater than or equal to 0.75 D was 40%. CONCLUSION: The prevalence of refractive errors, by the spherical equivalent, was 42.4% in this studied group.
