- ItemAcesso aberto (Open Access)Avaliação da restrição de participação e de processos cognitivos em idosos antes e após intervenção fonoaudiológica(Sociedade Brasileira de Fonoaudiologia, 2011-03-01) Magalhães, Ruth [UNIFESP]; Iorio, Maria Cecilia Martinelli [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To evaluate the restriction of participation in activities of daily living and cognitive processes in older adults, according to gender and age, before and after rehabilitation. METHODS: The study included 50 individuals, 23 females and 27 males. The elderly were divided into two groups: Group 1 to 24 individuals aged between 60 and 74 years (11 females and 13 males), Group 2 - 26 elderly aged above 75 years (12 of female and 14 male). The elderly were evaluated before one year after the adaptation of hearing aids through the HHIE test and MMSE. Bimonthly follow-ups were conducted a total of seven meetings, to ensure the effective use of the prosthesis. As for the statistical analysis were performed descriptive and statistical analysis. RESULTS: There was a reduction of the restriction of participation in social and emotional scales HHIE post-intervention in both groups, individuals of both genders. The MMSE test results were better in the post-intervention, regardless of gender and age of the participant. CONCLUSION: There is little perception of the restrictions on participation in activities of daily living after rehabilitation. Moreover, the elderly of different ages and both genders decreased cognitive constraints refer after hearing aid fitting

It is describe the case of a 26-year-old woman who presented sudden bilateral deafness, as a result of Cogan's syndrome - which also affects the vision. When the intervention process began, the patient had restricted orofacial reading, difficulty to communicate with her family, social isolation and depressive symptoms. The dynamic assessment based on interactivity and humanistic listening provided conditions for the patient to improve her communication, self-concept and social interaction. From this perspective, it was noticed that, with only ten sessions of intervention, therapeutic effects were operated, therefore breaking the classic assessment/intervention dichotomy at different moments.

Métodos: Trata-se de uma estudo retrospectivo, do tipo caso-controle, no qual foram avaliados 22 pacientes com surdez pós-lingual, submetidos à cirurgia de implante coclear, sendo 11 com suspeita diagnóstica de meningite bacteriana (casos) e 11 com outro tipo de etiologia (controles). Os pacientes foram avaliados quanto aos limiares auditivos, com a média tri-tonal, índice de reconhecimento de palavras e de sentenças em conjunto fechado e aberto, no período pré-implante coclear e após a ativação do mesmo (6, 12 e 24 meses). Resultados: Não foi observada diferença com significância estatística nos resultados audiológicos e no s testes de desempenho de fala entre os grupos, após a cirurgia do implante coclear. Conclusão: A surdez pós-meningite, por si só, não parece estar relacionado um pior desempenho com implante coclear.

Child development consists in gradually acquiring motor and psychocognitive abilities. Entering the symbolic world is decisive for the child to reach higher levels of linguistic complexity. AIM: to correlate symbolic play and aspects of child development in deaf children and in hearing children. METHOD: participants of this study were 32 children, of both genders, with ages between 2 and 6 years, age matched. All participants were submitted to the Evaluation of Symbolic Maturation and to the Denver Developmental Screening Test II. Sixteen participants presented moderate to profound sensory-neural hearing loss and composed the research group (RG); the remaining 16 children had normal hearing and composed the control group (CG). RESULTS: symbolism was observed in the play of 81.25% of RG and in 87.5% of CG. In the Denver Developmental Screening Test II, 100% of the RG was classified as being at risk. As for the CG, 94% of the children were classified as normal and the remaining 6% as being at risk (p<0.001). CONCLUSION: a similar performance was observed between the groups for symbolic play. However, in a qualitative analysis, the RG presented less complex symbolic play than the CG. It was observed that the RG presented a performance in the symbolic play compatible to its performance in aspects of personal-social, refined motor and gross motor control in the Denver Developmental Screening Test II.
- ItemAcesso aberto (Open Access)Competência de leitura em alunos surdos inclusos na rede regular de ensino(Universidade Federal de São Paulo (UNIFESP), 2011-01-26) Luccas, Marcia Regina Zemella [UNIFESP]; Goulart, Bárbara Niegia Garcia de [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Objective: To evaluate the efficacy of reading in hearing impaired or deaf students included in schools of the local regular education network. Methods: analytical observational study conducted with 35 students with sensorineural hearing loss of varying degree, with and without teaching aids, and 71 age- and series-matched listeners. All subjects underwent the Competence Test of Words Reading-TCLP 1.2, Picture Naming Test by Choice of Words-TFN 1.1 and Competence Test of Sentence Reading-SRT 1.2. Results: The deaf or hearing impaired students included in regular education network obtained different results listener students. In the test of Competence Test of Words Reading-TCLP, listeners obtained an average hit rate of 84.5%, while the deaf students obtained 68.8% of accuracy (p=<0,001). Regarding to the Picture Naming Test by Choice of Words-TFN, following average accuracy was observed: 91.6% for listeners, hearing impaired or deaf, 73%(p=<0,001). As for the Competence Test of Sentence Reading-TCLS, listeners achieved an average of 75.7% and the hearing impaired or deaf, 42.9% (p=<0,001). There was no statistically significant difference between deaf and hearing impaired students who attend the support and follow-up class at inclusion and the hearing impaired or deaf students in regular classes only (p=<0,001). Conclusions: The hearing students showed significant difference in reading competence in the three tests presented compared with deaf or hearing impaired ones. We can assert that the hearing students have a good rating on levels of correlation, while the deaf show regular classification. This result points to a failure in the education of the latter in the regular education network.

The prevalence of MIDD in the diabetes population ranges between 0.5-3.0% depending on the ethnic background. AIM: To examine the frequency and clinical features of diabetes associated with this mutation in Brazilian patients with glucose intolerance. METHODS: The study population comprised: 78 type 1 diabetic subjects (group I), 148 patients with type 2 diabetes (group II), 15 patients with either type 1 or type 2 diabetes and hearing loss (group III) and 492 Japanese Brazilians with varying degrees of glucose intolerance. DNA was extracted from peripheral blood leucocytes and the A3243G mutation was determined by PCR amplification and Apa 1 digestion. In some individuals DNA was also extracted from buccal mucosa and hair follicles. The 3243 bp mutation was found in three individuals, all from group III, resulting in a prevalence of 0.4%. These subjects had an early age of diagnosis of diabetes, low or normal body mass index and requirement of insulin therapy. In conclusion MIDD is rare in our population and should be investigate in patients with diabetes and deafness.

Esta pesquisa revisitou as bases teóricas que estruturaram a proposta de educação bilíngue para surdos da Rede Municipal de Ensino da capital de São Paulo, bem como os aspectos legais que nortearam a sua implementação. Propôs verificar os impactos da criação das EMEBS sobre o atendimento educacional oferecido aos surdos e as relações entre os documentos oficiais e a prática bilíngue dentro das escolas. Para tanto, houve a escuta dos atores envolvidos na proposta. A partir dos dados obtidos foram determinadas três categorias de análise para esta dissertação: a formação dos docentes, o lugar que a Libras ocupa na escolarização dos surdos e a relação dos educadores com os materiais produzidos para orientação e apoio da prática em sala de aula. Os dados revelaram a dificuldade de estabelecimento de uma identidade verdadeiramente bilíngue e o diálogo enfraquecido entre os documentos orientadores e as práticas. As formações oferecidas por SME pouco reverberam nas escolhas metodológicas dos professores e não se conectam com as reais necessidades das escolas. As práticas bilíngues se resumem a experiências isoladas, pois a maioria dos professores resiste em desenvolver a proficiência em Libras e insiste em repetir concepções oralistas que reduzem a Libras a um instrumento para o ensino do português. As escolas ainda estão em busca da constituição da identidade bilíngue paulistana diante da nova configuração do público atendido.
- ItemAcesso aberto (Open Access)Efeito de fármacos ototóxicos na audição de recém-nascidos de alto risco(Sociedade Brasileira de Fonoaudiologia, 2010-01-01) Camara, Marília Fontenele e Silva; Azevedo, Marisa Frasson de [UNIFESP]; Lima, José Wellington de Oliveira; Sartorato, Edi Lúcia; Universidade de Fortaleza; Universidade Federal de São Paulo (UNIFESP); Universidade Estadual do Ceará; Universidade Estadual de Campinas (UNICAMP)PURPOSE: To calculate the incidence of sensorineural hearing loss (SNHL), to verify if there is a causal association between the use of ototoxic drugs and SNHL, and to establish the frequency of genetic mutations related to SNHL in high risk newborns. METHODS: The study was a retrospective and prospective cohort research with 250 children. Data was gathered from subjects' charts and with their caregivers. Moreover, subjects were submitted to auditory evaluation with distortion product otoacoustic emissions, timpanometry, visual reinforcement audiometry, auditory brainstem response and transient otoacoustic emissions. The study of the genetic mutation 35delG, and the mitochondrial mutations A1555G and A7445G was essential to evaluate the possibility that SNHL had a non-syndromic genetic origin. The association between the medicine use and the occurrence of hearing loss had been analyzed. RESULTS: The incidence of SNHL in high risk newborns was 11.6%, and causal associations between SNHL and the drugs administered were: amikacin and cefotaxime (OR=5.35), cefotaxime and furosemide (OR=7.02), ceftazidime and vancomycin (OR=9.12). The frequencies of the mutation 35deIG and mitochondrial mutations A1555G and A7445G were, respectively, 0.8% and 0%. CONCLUSION: The incidence of SNHL in high risk newborns was high, showing an important causal relation with the use of ototoxic drugs and a small relation with genetic mutations.

And more specifically: to identify the difficulties and needs entailed by deafness before and after cochlear implantation; to understand the changes that the cochlear implant entails in daily life; and to identify coping strategies and the facilitating elements of this experience. Methods: The project was approved by the Research Ethics Committee of the institution (Opinion n ° 482,019). Qualitative research, using Symbolic Interactionism and Straussian Grounded Theory as the theoretical and methodological frameworks, respectively. A total of sixteen adult cochlear implant users, aged 18-60, who used the device for at least one year, participated in the study. Data collection instrument: semistructured interview, which covered questions about the trajectory from diagnosis to the present, inquiring about the changes caused by deafness and cochlear implantation in social relations, communication, family system, academic life and/or professional, in relationships, in leisure activities, and in personal projects. Results: The data analysis process led to the development of five categories that represent this experience. They are: Repercussions of deafness in adult life, Search for ways to solve problems, Facilitating factors in coping, Repercussions of cochlear implant use in adult life, and Difficulties after cochlear implantation. These categories make up a substantial theory represented by the process: SEEKING TO REGAIN INDEPENDENCE, AUTONOMY AND FREEDOM IN ADULT LIFE. Conclusion: Deafness profoundly impacts human social experiences, leading to changes and difficulties that affect the individual, family and society. However, the cochlear implant intervenes in a revolutionary way in the lives of its users, allowing them to live with a new perspective, to recover self-confidence and self-esteem, to communicate and interact more effectively, to feel more secure, to enjoy music and other types of entertainment, getting back past projects and make plans for the future. Thus fostering the recovery of the independence of adult life. In spite of this, the implanted person still experiences difficulties and limitations in the following aspects: agility to interact, reintegration into the labor market, going back to studying, localizing sounds spatially, experiencing prejudice and discrimination, using the phone, changes in balance and presence of tinnitus, costs with maintenance of the device, not being able to hear in situations where one needs to be without the IC, listening to music, understanding different types of entertainment, lack of family support, of assistive technology, and of continuity in psychological, social and auditory training rehabilitation. All these difficulties need to be identified and managed by the professionals who support the implanted person so that they can be overcome.

MethodWe conducted a cross-sectional study, with 78 hearing impaired individual diagnosed with sensorineural, symetric, postlingual hearing loss; and age between 12 and 70 years (mean=51.3±21.6; median=53), who speak brazilian portuguese as first language. We raised data about sociocultural status, hearing loss and hearing aids characteristics. The “Teste de Vocabulário por figura USP–Tvfusp92o”, in portuguese, was used to evaluate the receptive vocabulary. ResultThe hearing impaired individuals mean score in the recepetive vocabulary test was 82.9 (SD=26.0), showing significant correlaton with the educational level, reading habits and the use of a hearing aid. When compared with the test normative data, 51,3% of the hearing impaired scored as having poor or very-poor receptive vocabulary. ConclusionSociocultural factors, as educational level and reading habit, and the use of a hearing aid have a positive influence on receptive vocabulary. By other side, biological factors as age and hearing loss characteristics did not demonstrate this influence. Mostly of hearing impaired subjects had worst scores in the receptive vocabulary test when compared to normal-hearing individuals, from seven to ten years old.

METHODS: Case-control study composed by 32 children of both genders with ages between two and six years, paired by age: 16 hearing impaired with moderately severe to profound hearing loss without other organic dysfunctions (cases), and 16 children with normal hearing with no Speech-Language Pathology complaints (control). The evaluation and analysis of pragmatic abilities were carried out based on the ABFW-Pragmatics Test, following instructions of its own protocol. RESULTS: The average age of the studied children was four years (SD=1,3); there was a significant difference between cases and controls regarding the number of communicative acts per minute (p=0,001). The hearing impaired children presented fewer communicative initiatives than normal listeners, and the gestural communicative mean was used by 13 (81.3%) of the first group and by five (32.2%) of the latter (p=0,004). There was no difference between the groups regarding communicative intentions (p=0,465). CONCLUSION: The hearing impaired children were able to interact in contextualized situations using communicative functions similar to the listeners, and differed from them in the most used communicative mean.

No entanto, a surdez pode se configurar como uma identidade cultural diversa, uma perspectiva de mundo moldada pela língua de sinais (viso-espacial) e por tradições, valores e hábitos que formam a chamada cultura surda, heterogênea e em constante processo de mutação, sendo portanto a uma variação da diversidade humana. A escassez de profissionais de saúde mental capacitados em LIBRAS e o estigma relacionado à incapacidade dificultam o acesso dos surdos não oralizados a diversos serviços de saúde, especialmente no campo da saúde mental. Nesse contexto, a barreira da comunicação se torna um obstáculo significativo, prejudicando a qualidade do atendimento psicológico. Este artigo tem como objetivo problematizar o atendimento psicológico à comunidade surda com base em consultas a dissertações, teses e artigos sobre o tema. A pouca literatura encontrada sob essa temática revela lacunas, como por exemplo a falta de relatos de experiências de profissionais de saúde mental que atendem em LIBRAS (surdos ou ouvintes). A literatura aponta que o domínio da LIBRAS, associado ao conhecimento da cultura surda são essenciais para um cuidado mais sensível e eficaz no contexto psicoterapêutico. Há também relatos que avaliam que a presença de intérpretes como mediadores em sessões de terapia pode interferir negativamente nas questões éticas de sigilo e/ou criar desconforto e desconfiança nas pessoas surdas atendidas.
- ItemAcesso aberto (Open Access)Respostas falso-positivas na triagem auditiva neonatal universal: possíveis causas(CEFAC Saúde e Educação, 2011-04-01) Simonek, Maria Cristina Silva; Azevedo, Marisa Frasson de [UNIFESP]; Instituto Nacional de Educação de Surdos; Universidade Federal de São Paulo (UNIFESP)PURPOSE: to investigate the rate of false positive responses in a hospital - based newborn hearing screening program (NHS), their possible causes and solutions. METHODS: during the period we evaluated 1,110 regular nursery newborns, age 6 to 48 hours with Transient Evoked Otoacoustic Emissions (TEOAEs), Echocheck, Ilodynamics Ltd model., at the nurseries, before discharge. In case of absence of response, the ear canal was manipulated. Infants that failed the screening were retested fifteen days after birth. RESULTS: 50.09% failed the first trial, then the ear canal was manipulated, and it decreased to 24.41%. This group showed normal TEOAEs at the retest time; Mean time of hospital discharge was 42,27h, thought 93% cesarian delivery; 98% double room and 2% single room status. The mean age of the pass group was 24.14 h (± 10.21) and the fail group 19.19 hours (± 8.43). 66.12% had less than 24 hours on the moment of the test. 4.9% were occluded with vernix. CONCLUSION: hospital trend toward early discharge after routine deliveries forces the audiologist to carry out the screening before 48 hours. High number of false-positives in NHS, must be seen as a Hospital Administrative Problem not only as a biologic cause, like the vernix caseosum, and it is not related to the professional experience. Ear manipulation is strongly recommended.

The most common ocular findings in early Cogan's syndrome are bilateral, peripheral, subepithelial numular corneal opacities. The report presents a patient with the evolution of the classic form of Cogan's syndrome.

Other common findings are deafness, urinary tract and neurological disorders. It is an autossomic recessive disease, with incomplete penetrance and variable expressivity. The aim of this case report is to describe a patient who presents all the characteristics of Wolfram syndrome (DIDMOAD syndrome). JFP, African - American, 23 years old, presents with diabetes mellitus and insipidus, optic atrophy, deafness, peripheral polyneuropathy, autonomic neuropathy, neurogenic bladder, urinary tract dilation with recurrent infections, and azoospermia. Clinical examination, retinography, fluorescein angiogram, eletroretinography (ERG) and visual evocated potencial (VEP) revealed no retinal disorders and bilateral optic atrophy. Wolfram syndrome must be remembered in cases of optic atrophy in association with diabetes, urinary disorders, or any of the described alterations.
- ItemAcesso aberto (Open Access)Speechreading as a communication mediator(Sociedade Brasileira de Fonoaudiologia, 2014-02-01) Oliveira, Letícia Neves de; Soares, Alexandra Dezani [UNIFESP]; Chiari, Brasilia Maria [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Purposes:To compare the speechreading between individuals with hearing impairment and with normal hearing levels to verify the factors that influence the speechreading among hearing impaired patients.Methods:Forty individuals with severe-to-profound hearing loss aged between 13 and 70 years old (study group) and 21 hearing individuals aged between 17 and 63 years old (control group) were evaluated. As a research instrument, anamnesis was used to characterize the groups; three speechreading instruments, presenting stimuli via a mute video, with a female speaker; and a vocabulary test, to verify their influence on speechreading. A descriptive and analytical statistics (ANOVA test and Pearson's correlation), adopting a significance level of 0.05 (5%).Results:A better performance was observed in the group with hearing impairment in speechreading tests than in the group with hearing individuals. By analyzing the group with hearing loss, there was a mean difference between tests (p<0.001), which also showed correlation between them. Individuals with pre-lingual hearing loss and those who underwent therapy for speechreading had a better performance for most speechreading instruments. The variables gender and schooling showed no influence on speechreading.Conclusion:Individuals with hearing impairment had better performance on speechreading tasks in comparison to people with normal hearing. Furthermore, it was found that the ability to perform speechread might be influenced by the vocabulary, period of installation of the hearing loss, and speechreading therapy.

However, hearing impairment can compromise the acquisition of oral language to such an extent that appropriate use of these can be challenging. The objective of this study was to compare the use of nouns and verbs in the oral narrative of hearing-impaired and hearing children.DESIGN AND SETTING:Analytical cross-sectional study at the Department of Speech-Language and Hearing Sciences, Universidade Federal de São Paulo.METHODS:Twenty-one children with moderate to profound bilateral neurosensory hearing impairment and twenty-one with normal hearing (controls) were matched according to sex, school year and school type. A board showing pictures was presented to each child, to elicit a narrative and measure their performance in producing nouns and verbs.RESULTS:Twenty-two (52.4%) of the subjects were males. The mean age was 8 years (standard deviation, SD = 1.5). Comparing averages between the groups of boys and girls, we did not find any significant difference in their use of nouns, but among verbs, there was a significant difference regarding use of the imperative (P = 0.041): more frequent among boys (mean = 2.91). There was no significant difference in the use of nouns and verbs between deaf children and hearers, in relation to school type. Regarding use of the indicative, there was a nearly significant trend (P = 0.058).CONCLUSION:Among oralized hearing-impaired children who underwent speech therapy, their performance regarding verbs and noun use was similar to that of their hearing counterparts.
