METHODS: A qualitative approach was used to observe and interview seven nurses from a public hospital at São Paulo. RESULTS: Content analysis revealed two themes: a child's potential to develop autonomy and a child's incapability to exercise autonomy. FINAL CONSIDERATIONS: Few opportunities are given to children in order to develop their autonomy. As a rule, in conflicting situations nurses do not consider children decision-making; there is a disconnection between nurse's speech and professional practice.

This situation is worsened when she is a precautionary condition, which deprives her of freedom and of being able to interact with other hospitalized children. The possibility of playing has been recommended to reduce the stress arising from the hospitalization. Thus, this study was intended to understand the meaning of playing for the hospitalized child and in precaution. This is a qualitative research that used the Symbolic Interactionism as its theoretical benchmark. It was performed in a public children’s hospital in the city of São Paulo, with eight children aged between five and ten years, hospitalized in private and precautionary rooms. It´s project was approved by the Research Ethics Committee of UNIFESP under opinion nº 1.267.939/2015, and all ethical procedures were respected. The data collection occurred between April and September 2016 and was accomplished by means of participant observation, with the elaboration of a field diary, based on a playful interaction developed with the child during three consecutive days, and by means of a semi-structured interview mediated by the Story-Drawing Procedure with Theme. The data were analyzed by means of Qualitative Content Analysis by following the steps of codification, categorization, integration and description of the unveiled categories. The results revealed that, when playing, the child perceives herself as an active subject in the hospitalization process, having voice and placing herself as the main actor that can decide and dominate a situation; expresses her dreams and desires, including the desire to get rid of the confinement. They also showed that, during the three days of the playful activity, there was an evolution of the interactions of the child with the environment, with the researcher and with herself, thereby becoming calmer, quiet and happy, besides perceiving herself as a respected person. Furthermore, they highlighted that the child defines the hospital as a place of distress, restrictions and procedures, and nursing professionals as the only people who perform them; unveiled the importance and the benefits of taking part in a captivating playful activity, with an adult who is available to play, and the strategies developed by the child to take part in this activity, despite the difficulties and limitations that she has. One should emphasize that, as a defender of children’s rights, the nursing professional must engage in the promotion of the playful activity, incorporating it into his health care practice, aiming to establish the bond and to facilitate the performance of care actions, demystifying the image of the nurse as the professional who only performs procedures, enabling the expression of the child, making the hospitalization a less traumatic event and allowing her to continue being a child and to achieve the ability to get rid of the confinement for being hospitalized and in precaution.

Their underprivileged life conditions and restricted access to health care services are important determining factors. The objective of the study was to assess hospital morbidity and to identify factors associated to hospitalization of children under five years of age. METHODS: A data set derived from a cross-sectional study on health conditions of children under five years of age in the city of Embu, a county located in the metropolitan region of São Paulo, Brazil, was used. The inclusion criteria were one child per family (random selection). The exclusion criteria were missing data on any study variable. The sample size was 893 children. Data was collected using household interviews with mother or caretaker. Statistical analysis was performed using logistic regression models to identify factors associated with hospitalization. RESULTS/CONCLUSIONS: Sixty-five (7.7%) children were hospitalized. Of them, 41.5% were admitted with a respiratory tract disease, mainly due to an ARI (27.7 %). Factors associated to hospitalization included: low birth weight; perinatal problems; chronic illness; death of a sibling under the age of five; grandmother as day caretaker; living in overcrowded places, and mother's higher educational level.
- ItemAcesso aberto (Open Access)Identificação da vulnerabilidade da família na prática clínica(Universidade de São Paulo, Escola de Enfermagem, 2006-06-01) Pettengill, Myriam Aparecida Mandetta [UNIFESP]; Angelo, Margareth; Universidade Federal de São Paulo (UNIFESP); Universidade de São Paulo (USP)In this paper we present a report of a clinical situation in which we identified the vulnerability of the family, propitiating to pediatric nurses the use of this concept in their practice and intervention with families. The study was carried out with a family that was going through a situation of a child illness and hospitalization. The vulnerability was identified in the interactions of family members with the illness, with other family members and with the health staff, and served as a guide for planning the nursing assistance. The interventions led to the strengthening of the family to face the situation and, for pediatric nurses, the recognition of the importance of using the results of theoretical research in their professional practice.

Method It was used the descriptive qualitative approach. The data collection was conducted in three stages: presentation of theoretical content; engagement with families in the light of Watson’s theory; and semi-structured interview with 12 pediatric nurses. The interviews were analysed using inductive thematic analysis, being possible to form three themes: Recognizing a framework for care; Considering the institutional context; and Challenges in family’s relationship. Results The theory favored reflections about self, about the institutions and about nurses’ relationship with the family of the child, normalized by a consciousness toward caring attitudes. Conclusion In this process, it is imperative that nurses recognize the philosophical-theoretical foundations of care to attend the child’s family in hospital.

METHODS: Integrative review of literature in databases - national and international - with a qualitative approach. RESULTS: The 16 selected articles belonging to international journals were published between 1990 and 2005; they allowed the identification of the following topics: biopsychosocial manifestations of the child and his family; Biopsychosocial factors related to the manifestations of the child and his family; benefits of early discharge of the child; and, information and support needed. CONCLUSIONS: The interventions focused on the child and his family should be started in the hospital check-in in order to improve their ability to cope with the situation. There is need to enhance research on this topic in developing countries, including the prospect of the family as a unit.

The theoretical references were the Symbolic Interactionism and Vygotsky's theory on the child's symbolic play. The methodological referential was the Theory Based on Data. Eleven children between the ages of 3 and 6 took part in the survey. The strategies for data collection were: participative observation, interviews with the children mediated by the Therapeutic Toy, and interviews with the mothers. The analysis of the data made possible the construction of the technical model Growing up with the mother's protective presence, which puts in evidence the vulnerability, the child's force and the protection given by the mother to face the mystery and the fear of hospitalization.
