Quando lançada no organismo, essa indolamina controla o ciclo circadiano e outras funções do organismo, desempenhando um papel importante na manutenção do organismo e no tratamento de diversas patologias. Alguns estudos demonstraram a ação da acupuntura na produção de melatonina endógena. Porém, estes estudos são escassos e há diversas lacunas a serem esclarecidas quanto às metodologias empregadas e escolha dos pontos acupunturais. Objetivos: Estudar os possíveis efeitos da acupuntura na glândula pineal e na produção de melatonina em ratos. Métodos: Rattus norvegicus albinus (n=30) foram divididos em três grupos: controle (n=10), sham (n=10) e acupuntura (n=10). Os animais do grupo acupuntura receberam a terapia (estimulação manual do ponto Baihui (VG20) em tonificação), por duas semanas, excetuando-se os finais de semana. Durante o mesmo período, os animais do grupo controle foram somente manipulados e os animais do grupo sham, receberam a terapia acupuntural do ponto selecionado com sham needles. Foi feita a quantificação das concentrações de melatonina no plasma sanguíneo, pelo método de ELISA e feita a avaliação das possíveis alterações morfológicas e histoquímicas nas células da glândula pineal, pelos métodos de Hematoxilina e Eosina (HE), Kitoh e Matsushita e Hematoxilina Fosfotúngstica (PTAH). Resultados: A análise histológica evidenciou com maior frequência, a presença de heterocromatina periférica nos pinealócitos do grupo acupuntura, os astrócitos desse grupo apresentam corpo celular aumentado e prolongamentos citoplasmáticos espessos e uma concentração expressivamente maior de micróglias, quando comparado aos grupos controle e sham. Não foram observadas diferenças entre os grupos, na concentração plasmática de melatonina e no peso das glândulas pineais. Conclusão: O uso da acupuntura manual tonificante do ponto Baihui (VG20) ocasionou alterações na morfologia dos astrócitos e na concentração de micróglias da glândula pineal. Não houve alteração estatisticamente significante na concentração plasmática de melatonina no sangue circulante após a acupuntura. - ItemSomente MetadadadosAcupuncture for tension-type headache in pregnancy: A prospective, randomized, controlled study(Elsevier B.V., 2012-12-01) Silva, João Bosco Guerreiro da; Nakamura, Mary Uchiyama [UNIFESP]; Cordeiro, Jose Antonio; Kulay Júnior, Luiz [UNIFESP]; Rio Preto Med Sch; Universidade Federal de São Paulo (UNIFESP); Universidade Federal de São Paulo (UNIFESP)Acupuncture for tension-type headache in pregnancy: a prospective, randomized, controlled study.Aim: This study was undertaken to test, under real-life conditions, the effects of acupuncture on headaches during pregnancy and compare this with a group of patients undergoing conventional treatment alone.Methodology: Forty-three conventionally treated (lifestyle modifications and stretching) pregnant women were randomly allocated into two groups to routine care or acupuncture. the severity and disability caused by this pain and its affect on mood, sleep and work were assessed using a Numerical Rating Scale (NRS). the main end-point was the mean difference between the first and the last interview after eight weeks of treatment. Secondary end points were the changes over time in the NRS assessments of pain intensity, disturbances in mood, sleep and working and the use of medications.Differences between initial and final sessions were analyzed by a two-sample test. Changes over time in the NRS assessment were analyzed by the Fisher exact test. Mood's test for medians was used and a p-value < 0.05 indicated a significant difference.Results: All women completed the treatment. No important adverse effects related to acupuncture treatment or to pregnancy's outcome were reported. Significant improvements were demonstrated for pain at the end of the trial in the study group, 3.9 vs 1.7 difference in favor to acupuncture group (p < 0.05). This group also used less medication and had a greater improvement in mood and sleep when compared with the control group.Conclusions: This study suggests acupuncture alleviates tension-type headaches during pregnancy. - ItemSomente MetadadadosAcupuncture reduces immune-mediated pulmonary inflammatory lesions induced in rats(Karger, 2009-01-01) Katsuya, Elizabeth Marie [UNIFESP]; Pedreira de Castro, Maria Alice [UNIFESP]; Carneiro, Celia Regina Whitaker [UNIFESP]; Yamamura, Ysao [UNIFESP]; Silveira, Vera Lucia Flor [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: the aim of this report was to experimentally demonstrate the biological actions of acupuncture in an animal model of immune-mediated inflammation associated with a deposition of collagen. Materials and Methods: Male Wistar rats were sensitized by a subcutaneous implant of heat-solidified hen egg-white and divided into 4 groups: acupuncture, sham acupuncture, immobilized, and control. Acupuncture was initiated the day after sensitization and repeated twice a week for 3 weeks. the dorsal acupoints chosen were GV-14 and BL-13, the ventral acupuncture points were LU-1, CV-17, ST-36 and SP-6. the dorsal points were stimulated manually and the ventral ones by electroacupuncture. On day 14, animals were challenged through the tail vein with Sepharose (R) beads coupled with ovalbumin. One week later, animals were bled, plasma corticosterone concentrations were measured and the lungs were removed for histological evaluation. Results: Measurement of the areas of pulmonary lesion on hematoxylin-eosin stained slides showed a significant decrease (p < 0.05) in the inflammatory infiltrate in the acupuncture group, compared to the other 3 groups. Utilization of Litt and Picrosirius staining methods, in order to better visualize the infiltrate of eosinophils and the deposition of collagen, respectively, showed that both were much less intense in the acupuncture group. Corticosterone plasma levels were similar in all groups. Conclusion: Point-specific acupuncture treatment effectively reduced the inflammatory process and the deposition of collagen around ovalbumin-Sepharose beads intravenously embolized to the lungs of rats previously sensitized with the same protein that was administered subcutaneously.

MÉTODOS: Ensaio clínico randomizado, controlado, paralelo e aberto. Critérios de inclusão: mulheres submetidas à quimioterapia com paclitaxel a partir do 1º ciclo, tanto neoadjuvante quanto adjuvante e nos estádios clínicos de I a III. Critérios de Exclusão: mulheres com metástase à distância ou com doenças pré-existentes tais como: doenças reumatológicas, com limitações de movimentos dos membros e faziam uso de dispositivos para auxílio da marcha e as que realizaram acupuntura nos últimos três meses. Foi realizado um atendimento uma vez por semana, por dez semanas consecutivas. Divisão dos grupos: o Grupo Stiper (GS), técnica não invasiva, utilizada para estimular acupontos nos meridianos do corpo. Grupo Agulha (GA) técnica com agulhas, estéreis e descartáveis, para estimulação de pontos de acupuntura definidos para esse estudo. Grupo Cinesioterapia (GC) realizou exercícios de reabilitação em membros superiores e membros inferiores, adicionado ao treino de propriocepção e de equilíbrio. Os Grupos GS e GA, realizaram acupuntura nos mesmos acupontos: (B11Dazhu, SI14 Jianwaishu, CV12 Zhongwan, GB34 Yanglingquan, SP6 Sanyinjiao, LR3 Taichong), com frequência de 1 vez por semana, por 10 semanas seguidas e com duração de 30 minutos. Empregou-se o estesiômetro, para se avaliar a sensibilidade cutânea de membros superiores e inferiores. RESULTADOS: Selecionou-se 102 mulheres com diagnóstico de câncer de mama, sendo excluídas 39 pacientes, logo, foram incluídas e randomizadas 63 voluntárias que foram alocadas em três grupos GS (n=26), GA(n=18) e GC (n=19). As medidas dos pés (lados Direito/Esquerdo), apresentaram modificações nos três grupos, com variação de 1,00 até 2,00, o que corresponde a alteração de sensibilidade leve. Foram analisadas as medidas de não inferioridade, adotadas no grupo padrão de comparação que foi à Cinesioterapia. As medidas de sensibilidade dos Grupos GS e GA foram mensuradas pelo estesiômetro e comparadas com à cinesioterapia. Na Medida T1 D, o GS apresentou resultado maior no início. No entanto, após a intervenção, o GS apresentou resultado significativamente menor que o GC (p<0,01). As outras variáveis da sensibilidade, não apresentaram significância estatística, ou seja, demostrou equivalências entre GS e GA em relação à Cinesioterapia. O questionário FACIT-G analisou a qualidade de vida geral entre os grupos, pré e pós-intervenções, resultado sem significância estatística. O questionário Taxane total, mostrou piora significativa nas medidas pré e pós-intervenção, porém sem efeito sobre neuropatia. O questionário TaxS avaliou pré e pós-intervenção, com o valor p<0,03**. CONCLUSÕES: Conclui-se que as terapias experimentais: acupuntura com Stiper ou Agulha foram equivalentes à Cinesioterapia terapia adotada como padrão. A Qualidade de vida apresentou redução nos domínios, mostrando que autopercepção se altera durante a quimioterapia.

A qualitative research approach was used, with in-depth interviews. The results showed that acupuncture practiced within the public health services enabled interdisciplinarity, thereby integrating individuals' self-perception and their perception of context and allowing changes in the primary causes of diseases. Thus, contact with acupuncture as practiced in the health services of the municipal network of São Paulo may be able to contribute towards developing health promotion actions.

O presente estudo objetivou explorar as diversas fases que a acupuntura atravessou ate sua aceitacao no meio medico e insercao no Sistema Unico de Saúde no Municipio de São Paulo, tomando como base as experiencias de vida dos profissionais praticantes desta tecnica. Analisou, atraves das narrativas, como ocorre a mediacao entre a visao da Medicina Tradicional Chinesa e da Medicina Ocidental no espaco publico, bem como o posicionamento dos diferentes praticantes de acupuntura quanto a necessidade da regulamentacao desta pratica no Brasil. Utilizamos a Historia Oral de Vida como metodo, pois tal abordagem metodologica demonstrou ser a mais adequada frente aos objetivos propostos de acessarmos a dimensao das experiencias humanas envolvidas nesta trajetoria de lutas e desafios, possibilitando a construcao de uma historia mais humanizada, que concordou tambem com os objetivos mais amplos desta pesquisa que se insere na linha de pesquisa de Humanidades e Humanizacao em Saúde. A Historia Oral segue um rigoroso conjunto de procedimentos para a constituicao das narrativas, que garantem a etica e a validade das mesmas como documentos a serem analisados. Realizamos dez entrevistas, as quais foram realizadas seguindo o conjunto de procedimentos exigidos pela Historia Oral: gravacao das entrevistas; confeccao do documento escrito: transcricao, textualizacao e transcriacao; conferencia e validacao do documento escrito; analise; devolucao do produto. As informacoes contidas nas narrativas foram analisadas e, posteriormente, tomando como referencial a abordagem de interpretacao denominada imersao/cristalizacao, como sugerida por Borkan (1999), foram estabelecidas as categorias de analise, visando compreender as questoes centrais do trabalho. Os resultados confirmaram nossas expectativas de que a utilizacao da Historia Oral de Vida foi a abordagem mais adequada a este estudo. Atraves das historias de vida dos colaboradores foi possivel observar que: a escolha pela acupuntura esteve intimamente relacionada a crise do modelo medico ocidental; frente a crise da medicina e a emergencia de um novo paradigma na Saúde, a acupuntura vem exercendo um importante papel neste cenario, perpassando sua funcao central e inegavel de proporcionar beneficios a Saúde da populacao; a implementacao e a consolidacao da acupuntura constitui-se um complexo processo que, apesar de ser uma realidade cada vez mais presente, existe ainda um extenso caminho para sua implementacao e consolidacao no Sistema Unico de Saúde. Desta maneira, o interessante e uma proposta de complementaridade entre as praticas ocidentais e orientais, atraves do estabelecimento de um caminho intermediario de saberes e praticas construidas a partir de uma concepcao holistica, nao centralizada na doenca, mas sim no individuo.

We interviewed 175 physicians from the Course in Chinese Medicine and Acupuncture administered by the Research Center on Chinese Medicine at the School of Medicine of the Federal University in São Paulo (UNIFESP/EPM), using questionnaires consisting of open and closed questions. The resulting data were submitted to statistical analysis, and the answers were analyzed using the content analysis technique, seeking to unveil units of meaning. This process ena bled establishing general and specific categories that lent meaning to the discourses. Three categorical groups were obtained: professional enhancement, expanding one's life horizons, and understan ding the patient in a more comprehensive dimension. Our data also showed that physicians wish to integrate acupuncture with their practice of conventional medicine, within which acupuncture has been consolidated as a medical specialty.

In Brazil, this problem is even greater, since reference centers are scarce. In addition, it is estimated that up to 190,000 children and adolescents have some rheumatic disease. Since Pediatricians are responsible for the first care and eventual referral, we must use all the resources for a more targeted education for the main rheumatic diseases of childhood and adolescence. Objectives: To elaborate, implement and evaluate a virtual learning environment (VLE) online in Pediatric Rheumatology, aimed at Pediatric residents, analyzing their effectiveness and satisfaction indexes. Methods: A total of 92 pediatric residents of the first and second year of Hospital Infantil João Paulo II (n = 44) and Universidade Federal de São Paulo (n = 48) were invited to participate in the study. Residents were randomized into a case group (who responded to the pre-course test, attended the six virtual pediatric rheumatology modules, with selected topics and content drawn from expert experience, and then responded to the post-course test and a satisfaction questionnaire) and control group (who did not had access to VLE and responded to the pre-course test and, after 4 weeks, responded to the post-course test). Results: Forty-seven residents (51% response rate) completed their articipation in the project. In the case group (n = 24), the mean percentage of correct answers was 14.05% [7.28%; 20.82%] higher in the post-course test when compared to the precourse test (p = 0.000). In the post-course test, there was a significant difference (p = 0.045) between the case and control groups (n = 23), and the mean percentage of correct answers was 8.41% [0.17%, 16.65%] higher in the case group. In the assessment of satisfaction with VLE use, residents considered the design of the site attractive (79%), with information distributed logically and clearly (83.3%) and easy navigation (91%). They reported poor prior knowledge in Pediatric Rheumatology (91%), and agreed that there was good learning in Pediatric Rheumatology through the methodology (75%), as well as the use of AVA as a learning tool (91%). Regarding the themes of the teaching modules, they were considered readable (> 90%), adequate (> 90%) and objective (> 90%). Conclusion: The virtual learning environment in Pediatric Rheumatology has proved to be an effective teaching tool with high levels of satisfaction in its use. This tool can be useful for spreading knowledge in Pediatric Rheumatology, providing the Pediatrician with an adequate knowledge for the initial assessment and conduct of patients with childhood rheumatic diseases.

Os secundários; identificar dentre os dois protocolos (acupuntura e fotobiomodulação), qual é o mais eficiente em minimizar a perda de rendimento e de redução na carga de exercícios decorrentes da fadiga, identificar se após a aplicação dos protocolos os indivíduos conseguem desempenhar a mesma carga de trabalho anterior a sua aplicação, ou se o trabalho pode ser potencializado. Métodos: Este estudo foi quantitativo e experimental, com uma amostra composta por 72 indivíduos ativos (42 homens e 32 mulheres) idades entre 20 e 38 anos, média de 25,79 anos, média de altura de 172,01 cm, média de peso de 72,28 kg e IMC 24,37. Para avaliação da fadiga, antes e após a intervenção foi utilizado o protocolo de Bosco modificado, com 3 saltos com contra movimento sucessivos, sobre uma plataforma de força da marca EMG System do Brasil®, modelo Biomec 412, operada pelo software EMGlab2 do mesmo fabricante, para indução da fadiga utilizou-se de um protocolo de 100 saltos com contra movimento no solo, analisando o padrão do salto até a falha ou pela decisão do avaliado em interromper o teste indicando a fadiga. Após o protocolo de fadiga, os indivíduos foram submetidos aos protocolos de tratamento do grupo A acupuntura, grupo B acupuntura placebo, o grupo C controle e o grupo D fotobiomodulação. Resultados: Os dados demonstram haver diferença significativa antes e após a intervenção nos grupos A e D. Sendo possível verificar uma diferença entre a média das alturas dos saltos 1 e 2, de 2,60 cm e 2,45 cm, para um tamanho de efeito de 0.387 e 0.413 respectivamente, com p < 0,001 em ambos os grupos. A análise de variância (ANOVA) demonstrou que para o salto vertical houve diferenças estatísticas significativas intra grupos, com base na interação do grupo e fatores de tratamento [F (3,64): 24.304; p < 0.001; η²: 0.013], demonstrado na tabela 6. Comparações pareadas a posteriori indicaram que as intervenções tiveram efeitos positivos significativos nos índices de fadiga pós-intervenção nos grupos A e D. Conclusões: Concluímos, que a acupuntura e a fotobiomodulação são eficientes em minimizar os efeitos da fadiga, havendo um indicativo que estes protocolos de tratamento podem potencializar a performance muscular podendo haver um ganho em altura após a intervenção.
- ItemAcesso aberto (Open Access)Análise integrada de genes e miRNAs diferencialmente expressos em carcinomas de ovário seroso de alto grau de pacientes com distintas respostas à quimioterapia(Universidade Federal de São Paulo (UNIFESP), 2021) Melo, Natalia Cruz E [UNIFESP]; Rocha, Rafael Malagoli [UNIFESP]; Universidade Federal de São PauloOBJECTIVE: This study intended to evaluate the action of acupuncture on myelodepression in women with breast cancer during treatment with anthracyclines and quality of life, before and after chemotherapy. METHODS: The population of this study was composed of women newly diagnosed with breast cancer at the UNIFESP Mastology outpatient clinic and with indication for chemotherapy with anthracyclines. Patients were randomized into two groups: the Acupuncture Group (GA) and the Control Group (CG). All participants, before and at the end of treatment with anthracyclines and at the end of it, answered the quality of life questionnaire (FACT-G) and were monitored for leukocyte and neutrophil levels in peripheral blood as an indicator of the level of spinal depression. The GA underwent intervention with Acupuncture for up to 10 sessions, starting before the first infusion of chemotherapy and continuing throughout the treatment. The points were used: GV14, CV6, ST36, SP6, KI3, GB39. RESULTS: A total of 26 women were randomized into 2 groups: GA (10) and CG (16). Of these, 7 (26.92%) received a dense dose indication (according to the service protocol) and, therefore, received the granulocyte stimulating factor G- CSF from the first cycle. Thus, for the analysis of the protective effect of acupuncture on the myeloprotective medulla, 2 women from GA and 5 from GC were excluded. The Quality of Life (QOL) analysis was performed for the total sample and also in the subgroups that received G-CSF. The need for secondary prophylaxis with G-CSF occurred in 72.7% (08/11) in the control group and in 12% (08/01) in the acupuncture group (p-value = 0.009). Regarding quality of life (QOL), the initial and final FACT-G showed a difference between the groups, where the mean GA at the end rose from 80.68 to 90.12 (p-value = 0.004) and in the CG mean fell from 81.95 to 70.59 (p-value = 0.003) .In the analysis of subgroups that used G-CSF, the QoL of women in GA showed better QoL (p-value = 0.036). CONCLUSION: Based on the results presented, acupuncture showed efficiency in the prophylaxis of myelosuppression induced by chemotherapy with anthracyclines and increased the quality of life of women during treatment, including those who needed G-CSF.

Both procedures up-regulate c-Fos expression in several areas of the central nervous system, representing therefore a major pitfall for the assessment of c-Fos expression induced by electroacupuncture. Thus, in order to reduce the effect of acute restraint we used a protocol of repeated restraint for the assessment of the brain areas activated by electroacupuncture in adult male Wistar rats weighing 180-230 g. Repeated immobilization protocols (6 days, 1 h/day and 13 days, 2 h/day) were used to reduce the effect of acute immobilization stress on the c-Fos expression induced by electroacupuncture at the Zusanli point (EA36S). Animals submitted to immobilization alone or to electroacupuncture (100 Hz, 2-4 V, faradic wave) in a non-point region were compared to animals submitted to electroacupuncture at EA36S (4 animals/subgroup). c-Fos expression was measured in 41 brain areas by simple counting of cells and the results are reported as number of c-Fos-immunoreactive cells/10,000 µm². The protocols of repeated immobilization significantly reduced the immobilization-induced c-Fos expression in most of the brain areas analyzed (P < 0.05). Animals of the EA36S groups had significantly higher levels of c-Fos expression in the dorsal raphe nucleus, locus coeruleus, posterior hypothalamus and central medial nucleus of the thalamus. Furthermore, the repeated immobilization protocols intensified the differences between the effects of 36S and non-point stimulation in the dorsal raphe nucleus (P < 0.05). These data suggest that high levels of stress can interact with and mask the evaluation of specific effects of acupuncture in unanesthetized animals.

Pessoa/ Butantã (CSEB), in São Paulo, perceive the body in contact with Traditional Chinese Medicine (TCM) configured as a therapeutic of different medical rationality from Biomedicine. Thus, the initial hypothesis was that in the contact with acupuncture new forms of thinking the body, health and illness can be apprehended. The investigation was developed with an ethnographic approach in the clinic of acupuncture of the CSEB, from September 2008 to April 2009. The observations had revealed a heterogeneous group of professionals and patients (as to the age, gender, religion and territorial origin) in contact with the TCM. The majority of the interviews reveal that people´s contact with Traditional Chinese Medicine was given by personal experiences with distress, pain and suffering related to their bodies, having them appealed to the acupuncture as resource and, later, seeking its beddings. Diverse ways of thinking about ill / health and of using the body were identified, but always sustains by two opposites but complementary dimensions: health body and not health body. And tree forms of how to perceive the body also were identified: Ideas about the body, Reactions and Techniques of the Body. From the experience with TCM, people get in touched and embodied, partly, with a body conception that consider the energetic and invisible dimensions as formerly than the organic material. It also counts that, in this process, they had changed some daily habits and the way they perceived and used the body related with Meditation, Exercises and food provisions. Based in experiences and conceptions of body previous to the treatment, they had disclosed very distinct senses for the experience of each one. For example, the body was defined as ―a sanctuary‖, as ―a structure‖, ―the soul‘s car‖, among others.

MÉTODOS: A população deste estudo foi composta por mulheres recém diagnosticadas de câncer de mama do ambulatório de Mastologia da UNIFESP e com indicação de quimioterapia com antracíclicos. As pacientes foram aleatorizadas em dois grupos: o Grupo Acupuntura (GA) e Grupo Controle (GC). Todas as participantes, antes e ao final do tratamento com antracíclicos, responderam ao questionário de qualidade de vida (FACT-G) e tiveram acompanhamento dos níveis de leucócitos e neutrófilos no sangue periférico como indicador do nível de depressão medular. O GA sofreu intervenção com Acupuntura por até 10 sessões, iniciando antes da primeira infusão de quimioterápicos e prosseguindo ao longo do tratamento. Foram usados os pontos: GV14, CV6, ST36, SP6, KI3, GB39. RESULTADOS: O total de 26 mulheres foram aleatorizadas em 2 grupos: GA (10) e GC (16). Destas, 7 (26,9%) receberam indicação de dose densa de acordo com o protocolo do serviço e, portanto, receberam o fator estimulante de granulócitos (G-CSF) desde o primeiro ciclo. Assim para análise do efeito protetivo da acupuntura sobre a medula, foram excluídas 2 mulheres do GA e 5 do GC. A análise da Qualidade de Vida (QV) foi realizada para a amostra total e também nos subgrupos que receberam G-CSF. A necessidade de profilaxia secundária com G-CSF ocorreu em 72,7% (08/11) no grupo controle e em 12% (01/08) no grupo acupuntura (p-valor=0,009). Em relação à qualidade de vida (QV), o FACT-G inicial e final mostrou diferença entre os grupos, onde a média do GA ao final subiu de 80,68 para 90,12 (p-valor = 0,004) e no GC a média caiu de 81,95 para 70,59 (p-valor=0,003). Na análise de subgrupos que utilizaram G-CSF, a QV das mulheres do GA apresentaram melhor QV (p-valor=0,036). CONCLUSÃO: Com base nos resultados apresentados a acupuntura mostrou eficiência na profilaxia da mielossupressão induzida pela quimioterapia com antracíclicos e aumentou a qualidade de vida das mulheres durante o tratamento, inclusive das que necessitaram de G-CSF.

Considering the wide clinical variety of this morbidity (extension and different presentations), there is an interest in studying the effect of acupuncture on the treatment of pain related to muscle injury in an identical model. Microsystem acupuncture is a practical, economical, fast-acting therapeutic resource with few side effects. Method: This study is a clinical trial in which 22 patients undergoing anterior cruciate ligament reconstruction (ACLR) surgery were divided into two groups. Group 1 participants received established rehabilitation treatment. Participants in group 2 received acupuncture treatment using nasal microsystem and cranial suture microsystem techniques, in addition to the rehabilitation program, to relieve muscle pain in the graft harvesting region. Participants were assessed for pain outcome by the Numerical Pain Scale (NPS) from the first to the sixth postoperative week. Results: Both groups showed a statistically significant reduction in pain as treatment progressed; however, a better outcome was evident in the acupuncture-treated group compared with the untreated group from the second to the fourth week (p<0.001). There was also a significant reduction in pain score before and immediately after the acupuncture session on the fifth postoperative day (p<0.001). Conclusion: The results of the present study suggest that the addition of acupuncture treatment with nasal microsystem and cranial suture microsystem techniques to the established rehabilitation treatment has brought benefits in reducing thigh muscle pain in the early postoperative period, being effective in short term.

Methods: A Randomized Double-Blind Clinical Trial With 50 Patients Was Conducted. One Group (23 Subjects) Received True Acupuncture (Av) And The Other (27 Subjects) Received Sham Acupuncture (As). Visual Numeric Scale (Vns) And The Shoulder Range Of Motion Were Evaluated Before And After Treatment. Furthermore The Motion Limitation Was Evaluated By A Questionnaire (Subjective Assessment) And By An Independent Examiner (Objective Evaluation). Results: There Was A Statistically Significant Decrease In Shoulder Pain In The Av Group (P <0,001) When Compared To Sham Acupuncture. SETTING: Department of Physiology, Sleep Division of Department of Psychobiology, Chinese Medicine Division and Department of Nephrology, Federal University of São Paulo, Brazil. INTERVENTIONS: Acupuncture applied in true acupoints accompanied by needle manipulations or electroacupunture 2 hz and 10 Hz for 30 minutes in all patients, except control group pacients. DESIGN: In a randomized, placebo-controlled and single blind study, fourty-three patients with an apnea-hypopnea index (AHI) of 15 to 30 per hour were randomly assigned to 4 groups: control group, MA group, EA 2Hz group and EA 10Hz group. The control group received no intervention. The MA group, EA 2Hz group and EA 10Hz group received treatment for five weeks (02 acupuncture aplications per week). All the patients were evaluated pre-treatment and pos-treatment by polissomnography recordings and assessment of serotonin levels. The 5HT measurement was made with a HPLC-ED (Liquid Chromatografy coupled with Electrochemical Detection). MEASUREMENTS AND RESULTS: 1 – Immediate effect: when compared before and after treatment, the AHI (P=0,006; P=0,009), respiratory events (P=0,012; P=0,005), microarousals (P=0,046; P=0,05) and mean O2 saturation (P=0,006; P=0,035) showed significant improvement in both the manual and electroacupuncture 10 Hz respectively. The apnea index (P=0,028) and the hipopneia index (P=0,046) showed significant improvement only in EA 10 Hz group. There were no significant changes in the electroacupuncture 2 Hz as well as in the control group when compared before and after (first and second for controls) PSG recordings. When compared each group and control group, AHI significantly decreased only in the EA 10 Hz. 2 – Mediate effect: When compared before and after treatment, sleep onset (P=0,011;P=0,005), AHI (P=0,011;P=0,006), apnea index (P=0,042;P=0,005), respiratory events (P=0,011;P=0,009) and microarousals (P=0,011;P=0,021 ) significantly improved in MA and EA 10 Hz respectively. Hipopnea-index (P=0,021) decreased only in the EA 10Hz. Latency for sleep onset (P=0,005;P=0,005) decreased in both EA 2Hz and control group. Except for this change, there were no other significant changes in EA 2 Hz, or in control group after treatment. When compared to the control group the only significant differences were AHI and microarousals that were decreased in EA 10 Hz group (P=0,004;P=0,008). As concerned to the levels of seric serotonin we could not evidence an involvement of this neurotransmitter (as measured in blood) with the immediate effects of acupuncture for OSHA (MA vs. controls [P=0,7674], EA 2 Hz vs controls [P=0,9849]) despite a tendency for a difference for EA 10 Hz as compared to controls (P=0,1042). With regard to an effect of mediate acupuncture in moderate OSHA we found that seric serotonin levels were signficantly different form controls for MA (P=0,0067) with a tendency for group EA 10 Hz (P=0,0984). The lack of more robust differences for seric serotonin levels might have been influcenced by a smaller numeber of subjects as that required for this sort of evaluations. Conclusions: MA and EA 10 Hz have greater efficacy as compared to EA 2 Hz in improving AHI and other respiratory events in patients with moderate OHSA. This effect was shown to hold for both in the immediate (1 acupuncture session) as in the mediate (10 sessions) acupuncture application. The preliminary serotonin results indicate a possible mechanistic link between acupuncture treatment and its beneficial effects for the treatment of patients with moderate OHSA.

Acupuncture has been increasingly used and recognized as a complementary medical treatment. Some studies on gastrointestinal motility are available; however, acupuncture effect on esophageal motility is still elusive due to the lack of studies with adequate methodology. This study aims to evaluate acupuncture effect on esophageal motility. Methods: We studied 16 (50% females, mean age 26 years) volunteers. No individual underwent acupuncture sessions previously. All individuals underwent high resolution manometry. The test was performed in 3 phases: basal measurements, 20 minutes after acupuncture stimulation of the gastrointestinal point (ST36) or 20 minutes after acupuncture stimulation of a sham point (5 cm medial to ST36) (crossover). ST36 or sham points were alternated in order based on randomization. Lower esophageal sphincter (LES) resting and residual pressure; distal latency (DL) and distal contractility integral (DCI) were recorded. All tests were reviewed by 2 experienced investigators blinded to the acupuncture point. Results: LES resting pressure was significantly reduced after acupuncture (p=0.015, Wilcoxon signed-rank test). DL was significantly increased after acupuncture (either Sham or ST36) as compared to basal measurement. Conclusion: Our results showed that acupuncture on the digestive point decreases LES basal pressure. Acupuncture may be an alternative treatment to spastic disorders of the LES.

[UNIFESP]; Mello, Luiz Eugenio Araujo de Moraes [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Despite its ancient use as a therapeutic tool to treat several ailments, acupuncture still faces the challenge of scrutiny by Western science both in terms of its efficacy and in terms of the characterization of its effects and mechanisms of actions underlying these effects. We investigated under well-controlled and carefully characterized conditions the influence of electrical stimulation of acupuncture points ST-36 (Zusanli) and SP-6 (Sanyinjiao) on the myoelectric activity of the small intestine of 38 adult male Wistar rats. Electrical recordings obtained by means of four electrodes chronically implanted in the small intestine were used to assess the effects of acupuncture (electroacupuncture stimulation set at 2 Hz, intermittent stimulation, 1 V, for 30 min). Immobilization of the animals was associated with a consistent decrease (-8 ± 7%) in the myoelectric activity of the small intestine as measured by means of the root mean square. Conversely, acupuncture was able to significantly increase (overshoot) this activity compared to baseline (+44 ± 7%). In contrast, immobilized animals subjected to sham acupuncture had only modest (nonsignificant) increases in myoelectric activity (+9 ± 6%). Using carefully controlled conditions we confirmed previous noncontrolled studies on the ability of acupuncture to alter intestinal motility. The characterization of the topographic and temporal profiles of the effects observed here represents a basis for future dissection of the physiological and pharmacological systems underlying these effects.
