Abstract Introduction: Common Variable Immunodeficiency (CVI) patients presenting diarrhea and weight loss probably have impaired nutrient ingestion and absorption. Objectives: To compare vitamin A and zinc levels in CVI patients and healthy controls and also compare those levels with gastrointestinal (GI) symptoms and bacterial translocation. Methods: CVI patients from the Immunology outpatients clinic from the Federal University of Sao Paulo and age and gender-matched healthy controls underwent nutritional and laboratorial evaluation with CBC, C reactive protein (CRP), lipopolysaccharide (LPS), soluble CD14 (sCD14), retinol, beta carotene, serum and erythrocyte zinc. Results: Seventeen patients (mean age 28.54y) and 17 controls were included in the study. Retinol levels were lower in patients compared to controls: 1.99 ( }0.67) and 2.72ƒÊmol/L ( }0.96), respectively, p=0.014. Likewise, serum zinc levels were 50.0 (50-100) in the patients and 100.0ƒÊg/dL (50-150) in the control group, (p=0.020). For erythrocyte zinc the levels were 37.32 ( }10.51) for patients and 44.91ƒÊgZn/gHb ( }7.67) for controls, p=0.045. CRP levels were higher among patients: 4.99 (0.15-34.51), compared to controls: 0.55mg/L (0.17-6.06), p=0.004. There was no difference in the translocation marker levels between the groups. A tendency of lower retinol was seem in those patients with severe GI symptoms compared with those with mild/absent symptoms and health controls. Conclusions: CVI patients presented significant lower retinol and zinc levels and higher CRP levels than the healthy controls with no evidence of bacterial translocation. Nonetheless, those patients with severe GI symptoms tend to have lower retinol levels than the other patients.

Introducao: Pacientes com Imunodeficiencia Comum Variavel (ICV), que apresentam diarreia e perda de peso, provavelmente tem comprometimento da ingestao e absorcao de nutrientes. Objetivos: Comparar as concentracoes sericas de retinol e beta caroteno e sericas e eritrocitarias de zinco em pacientes com ICV com individuos saudaveis e, nos pacientes, comparar essas concentracoes com: manifestacoes gastrintestinais (GI) e translocacao bacteriana. Metodos: Pacientes com ICV do Ambulatorio de Imunologia da Universidade Federal de Sao Paulo e controles sadios, pareados por sexo e idade, foram avaliados nutricional e laboratorialmente: hemograma, proteina C reativa (PCR), lipopolissacaride (LPS), CD14 soluvel (sCD14), retinol e beta caroteno sericos, zinco serico e eritrocitario. Resultados: Foram incluidos 17 pacientes (media de idade: 28,54 anos) e 17 controles. As concentracoes de retinol foram mais baixas nos pacientes do que nos controles: 1,99 ( }0,67) e 2,72ƒÊmol/L ( }0,96), respectivamente, p=0,014. O mesmo ocorreu com zinco serico: 50,0 (50-100) nos pacientes e 100,0ƒÊg/dL (50-150) nos controles (p=0,020), e eritrocitario: 37,32 ( }10,51) e 44,91ƒÊgZn/gHb ( }7,67), respectivamente, p=0,045. As concentracoes de PCR foram mais altas nos pacientes 4,99 (0,15-34,51) do que nos controles 0,55mg/L (0,17-6,06), p=0,004. Houve tendencia de reducao da concentracao serica de retinol nos pacientes com sintomas GI moderados/graves quando comparados com aqueles com sintomas mais leves e controles. Conclusoes: Pacientes apresentaram concentracoes de retinol e zinco mais baixas e de marcador de inflamacao mais elevadas que controles, sem evidencia de translocacao bacteriana. Entretanto, aqueles com sintomas GI mais graves, tendem a apresentar concentracoes ainda mais baixas de retinol.
